Warm Handoff and Recovery Support Services

WARM subrecipients provide peer recovery support services using a warm handoff model to individuals with opioid use disorder or other substance use disorder who are in a housing transition.

This includes:

Pending or recently discharged from a jail, prison, hospital, or inpatient treatment.
Transitioning within the justice system (e.g., monitored community release or completing probation or parole).
Currently experiencing homelessness or transitioning from homelessness into a housing setting.
Exiting a recovery residence (e.g., peer-run recovery residence or recovery home).

WARM subrecipients are expected to:

Establish relationships, agreements, and protocols to facilitate the warm handoff model.
Conduct outreach, engagement, and educational activities appropriate for the identified population.
Provide overdose education and naloxone distribution services.
Offer warm handoff services to treatment and recovery support services, in accordance with the program model.
Initiate delivery of housing support services to participants.
