CORS Subaward Announcement

Advocates for Human Potential (AHP) is pleased to announce 14 organizations that have been awarded funding for the initial period of performance as the Community Outreach and Recovery Support (CORS) subrecipients. In accordance with the Illinois Opioid Allocation Agreement, the Illinois Opioid Remediation Advisory Board recommended and the Governor’s Opioid Overdose Prevention and Recovery Steering Committee approved $15,000,000 over a period of 3 years for CORS services in the State of Illinois.

The purpose of this subaward is to deliver support services to individuals with opioid use disorder (OUD) and other substance use disorders (SUDs) experiencing homelessness or housing instability. The CORS subrecipient will develop and implement services provided by peer support workers (PSWs) or persons with lived experiences (PLEs). Activities include:

  • Collaboration with community partners
  • Peer and recovery coaching to promote recovery from OUD/SUDs
  • Offer connections to treatment, housing, recovery and other supports

The CORS subawardees include the following organizations.

Chicago – Black Men United (BMU) is a National Community Service organization that focuses on the restoration and rebuilding of Black communities across America under the leadership of Black Men.

Belvidere – The Boone County Health Department provides a variety of services related to public health, clinics, and environmental concerns.

Charleston – Hour House is a community-based, not-for-profit corporation offering services to address substance use disorders and other therapeutic needs.

Springfield – Fifth Street Renaissance provides services based on compassion, empowerment, and respect for the diversity of those living with or affected by HIV/AIDS and those experiencing or at risk of homelessness.

Chicago – Gateway offers a full continuum of care across a wide range of clinical settings, including outpatient, residential, day treatment and aftercare at locations throughout Illinois. CORS services will be located in St. Clair County.

Peoria – Human Service Center (HSC) – Trillium Place offers comprehensive addiction and mental health services in central Illinois.

Chicago – Haymarket Center is a designated Federally Qualified Health Center (FQHC) Look-Alike and provides comprehensive healthcare services, including treatment for substance use and mental health disorders.  

Round Lake – Nicasa Behavioral Health Services is a behavioral health and social service agency that has been serving the residents of Lake County since 1966.

LaSalle – The Perfectly Flawed Foundation is a grassroots community-based harm reduction and recovery community organization that provides services and support related to substance use, mental health and addiction.

Rockford – Rosecrance is a private, not-for-profit organization offering comprehensive, nationally accredited, evidence-based addiction treatment and mental health treatment for children, teens, young adults and adults.

Addison – Serenity House hosts men’s and women’s residential programs and offers, treatment,  individual counseling, group counseling, family support including family nights, and sober fun activities.

Eldorado – The Stress & Trauma Treatment Center, Inc. provides screening and assessment, treatment planning, care coordination, diagnosis, and therapeutic behavioral health services.

 

Chicago – Thresholds provides healthcare, housing, and hope for thousands of persons with mental illnesses and substance use disorders in Illinois each year.

Chicago – Trilogy provides an array of essential services and ongoing support so that persons with behavioral health concerns can live independently and thrive in our community.

AHP administers the Regional Care Coordination Agency (RCCA) grant as authorized by CSFA 444-26-3078. This subaward depends on the availability of funding for the Illinois Opioid Remediation State Trust Fund through the Illinois Department of Health Services (IDHS), for the express purpose herein.
