Warm Handoff and Recovery Support Services

WARM subrecipients provide peer recovery support services using a warm handoff model to individuals with opioid use disorder or other substance use disorder who are in a housing transition.

This includes:

  • Pending or recently discharged from a jail, prison, hospital, or inpatient treatment.
  • Transitioning within the justice system (e.g., monitored community release or completing probation or parole).
  • Currently experiencing homelessness or transitioning from homelessness into a housing setting.
  • Exiting a recovery residence (e.g., peer-run recovery residence or recovery home).

WARM subrecipients are expected to:

  • Establish relationships, agreements, and protocols to facilitate the warm handoff model.
  • Conduct outreach, engagement, and educational activities appropriate for the identified population.
  • Provide overdose education and naloxone distribution services.
  • Offer warm handoff services to treatment and recovery support services, in accordance with the program model.
  • Initiate delivery of housing support services to participants.
