Warm Handoff and Recovery Support Services (WARM)
At a Glance
- Warm Handoff and Recovery Support Services (WARM) funding will allow organizations to provide peer recovery support services delivered using a warm handoff model to individuals with opioid use disorder (OUD) or polysubstance use disorder who are experiencing a housing transition.
- Non-profit, for-profit, and tax-exempt organizations throughout Illinois, particularly in areas disproportionately affected by the opioid crisis, are encouraged to apply.
- A total of $6,000,000 is available to be awarded over a period of 3 years to a minimum of 7 organizations. Applicants may apply for up to $300,000 per period of performance.
- The anticipated funding periods for this NOFO are as follows:
- (1) October 1, 2024 – June 30, 2025
- (2) July 1, 2025 – June 30, 2026

Application Materials
Information about the Notice of Funding Opportunity is organized into four main topics: the NOFO, the application, the budget, and FAQs.
1. NOFO
The NOFO includes all program requirements and terms and conditions related to the award.
2. Application
Applications are accepted via SurveyMonkey Apply (SMA), an online application platform.

The Budget Workbook must be uploaded as an attachment in the online application.
